The questionnaire may have parts that are irrelevant, we ask you to fill in as much information as possible so we can help tailor support you and your puppy or young dog. I can only offer training advice and compile a plan following an initial assessment of your dog. This is to understand more about the context, environment, emotion and motivation behind behaviour you require support with. The questionnaire is private and confidential and will not be shared with any third party.

 

There is a permission box to seek advice from other professionals that are accredited, however personal information will not be shared about yourself, only the specific case and your dog that requires support. Often colleagues within the dog training and behaviour industry work together sharing knowledge. This benefits you, the owner and your dog as we work together.

A veterinary form may need to be completed by your vet, which will be emailed to you to forward to your vet. This is to confirm that your vet is happy for your puppy or young dog to move forward with training health wise. Sometimes puppies, like dogs may experience an illness or injury, and we want your puppy to enjoy and feel comfortable while learning.
